Below is a list of patient forms that are FILLABLE and/or printable on your computer. For your convenience, we recommend that you complete these forms prior to your appointment.
IMPORTANT: ALL patients MUST fill out the GENERAL INTAKE FORM as well as the intake form for each therapy they intend to use at the clinic
Example: If you are booked in for a massage, you must fill out the General Intake Form AND the Massage Intake Form.
